Evidence tier: live bench with an authoring blocker. This page reflects an attempted live build on the published WP Quiz Showcase subsite, plus captured WordPress.org directory metrics and vendor documentation. The native quiz builder accepted input but failed to save or publish, so no front-end quiz attempt was completed. See the project’s evidence-tier explanation for what that permits.

Forminator is distributed through the WordPress.org plugin directory and was installed and activated on the /forminator/ subsite of the showcase.

Pricing and source

Forminator’s core plugin is free from wordpress.org. WPMU DEV-hosted services and Pro features exist separately; no paid package, trial or nulled code was installed during this audit. The live behavior described below comes from the free tier as published on the showcase bench.

Documented funnel shape versus live proof

The directory documentation and in-plugin UI describe Forminator’s quiz funnel as:

  • Knowledge Quiz — a scored quiz with right/wrong answers.
  • No Wrong Answer / Personality Quiz — an outcome quiz that maps answers to a final personality or recommendation result.
  • Collect Leads — an automatically generated Forms-module lead form attached to the quiz.
  • Integrations — marketing and storage connectors that the UI marks as Pro-gated.

What was actually proven live is narrower. The Knowledge Quiz builder loaded, accepted the canonical Q4 question ("What is the most abundant gas in Earth's atmosphere by volume?") with four answer choices, and displayed it in the native question list. When Publish was clicked the builder returned the error Something went wrong while saving your quiz; the module stayed in Draft / Unsaved state and the shortcode remained [forminator_quiz id="undefined"]. A direct WordPress post-type query confirmed that no quiz module was persisted on this subsite. The Personality Quiz and Collect Leads UIs were inspected, but the same save failure meant no lead quiz or outcome quiz could be persisted either. No fabricated shortcode or substitute HTML quiz was used.

Because nothing persisted, no front-end benchmark attempt could be completed. The coverage table below therefore records every Standard Ten question as unsupported on the live bench, not as a claim about the plugin’s documented capability.

The builder UI exposed Pro-gated markers for advanced marketing integrations and for duplicate/export workflows around lead-enabled quizzes. These were recorded as observed capability risks, not worked around.

Runtime measurements

A measurement was captured at https://wpqp.loca.zone/forminator/benchmark-core/ under the project’s measurement protocol on 2026-08-03. Because the quiz did not persist, this page does not represent a rendered working quiz; it reflects whatever the subsite published at that path once the authoring attempt failed.

Signature feature walkthrough

The signature demo was intended to exercise Forminator’s personality / outcome quiz plus its native Collect Leads step on https://wpqp.loca.zone/forminator/signature/. The builder UI for Personality Quiz and for the auto-generated lead form was inspected, but the same Something went wrong while saving your quiz error prevented any signature quiz from being persisted or rendered. The documented funnel (outcome mapping + lead capture) was therefore not proven live.

Catches and caveats

  • Save/publish failure on the live bench: The native Knowledge Quiz builder accepted Q4 input but could not save. The exact error was Something went wrong while saving your quiz; the shortcode stayed id="undefined" and no quiz post was created. This is a live-bench finding, not a documentation claim.
  • Lead-quiz operational limits: Official wordpress.org support threads state that lead-enabled quizzes could not be exported or duplicated and had to be recreated. This matches the Pro-gated duplicate/export markers observed in the builder.
  • Form-builder first, quiz second: The plugin’s identity and largest feature surface are forms, payments and polls; the quiz module is a smaller sibling. Quizzing depth (question types, scoring logic, result reporting) is therefore more constrained than in dedicated quiz plugins.
  • No jQuery on the measured page: The captured Core-Six page did not load jQuery, but because no quiz rendered this signal is not comparable to the other roster plugins.

Coexistence lab

On the /coexist/ lab page Forminator contributed one top-level admin menu item (Forminator) to the shared wp-admin menu. No console error or debug-log entry was attributed to Forminator during the logged-out coexistence load.

Using the project’s rubric, Forminator scored 69.28 / 100 for the free tier:

The maintenance-and-trust score is high because of the large install base, recent update, strong rating volume and tested WordPress 7.0.2 compatibility. The quiz-depth score is low because the live bench could not express even the Core Six; the UX score reflects the polished builder interface that was observed before the save failure.

Verdict

Forminator is a polished, widely adopted form builder whose quiz module looks attractive for lead-capture and simple personality funnels, but the free-tier quiz save/publish path failed on this bench before any question reached the front end. The documented shape (knowledge quiz + outcome quiz + native lead form) is plausible and well presented in the UI, yet it was not proven live here.

If the save failure is reproducible on a user’s own site, Forminator cannot currently be relied on as a free quiz solution. If it saves successfully elsewhere, it is best suited for lightweight scored or outcome quizzes that feed directly into a lead form, not for rich assessment logic or advanced question types. For the WP Quiz Showcase roster, the live evidence stops at the builder; everything beyond that remains documented intent, not demonstrated behavior.
